Associate Director/Clinical Coordinator

The Associate Director will work with the Administrator to assure appropriate staff, programs, and services are available to residents on a 24 hour/day, seven day per week basis, with the goal of enabling residents to live permanently at Frederic Fleming House as safely and independently as possible. The Associate Director will serve as the Clinical Coordinator of Frederick Fleming House.


  1. With the Administrator, develop the program budget, monitor program expenses, monitor financial record keeping, and prepare any required financial reports.
  2. With the Administrator, develop and review policies and procedures for the facility.
  3. Report to and serve as liaison with all regulatory bodies including but not limited to DOH, DMH and Quality of Care Commission.
  4. With the Administrator, assure compliance to the requirements of

all regulatory bodies, included but not limited to DOH, DMH, Quality of Care Commission, and WSFSSH.

  1. With the Administrator, assure the provision of on-site psychiatric

care through PPOH.  Meet with PPOH staff to discuss individual resident need, and provide follow-up on psychiatric interventions.

  1. On-call responsibilities as scheduled.
  2. Act as the Administrator when the Administrator is absent.

Clinical Coordinator:

The Associate Director/Clinical Coordinator is responsible for the supervision of the provision of comprehensive clinical and social services to the residents of the facility. The Director of Clinical and Social Services for WSFSSH will provide clinical supervision for the Associate Director/Clinical Coordinator.


  1. In conjunction with the Administrator, hire social work and case work staff.
  2. Provision of clinical and task supervision for all social workers and case managers.  Supervision will focus on client assessment, development of a service plan, implementation and ongoing evaluation of service plan.
  3. Monitor case records to insure that all required documentation, including case notes, service plans, and psycho-socials are completed and updated in a timely manner.
  4. Maintain a small caseload.
  5. Provision of comprehensive social services for residents on caseload including assessment, development of a service plan, implementation and ongoing evaluation of service plan.
  6. Outreach to potential residents, initial interview of all applicants, and completion of all documentation regarding admissions.
  7. In conjunction with the Administrator, make decisions regarding admission and discharge of residents.
  8. Completion of all necessary forms/paperwork for discharged residents.
  9. Completion of all required statistical reports (DOH, DMH, etc.).
  10. Provision of crisis intervention services.
  11. Convene scheduled social service team meetings.
  12. Conduct scheduled case review meetings that will include the Administrator, the social service staff, and other staff as required.
  13. Develop and provide in-service training for Fleming House staff.


  •   LMSW required, LCSW strongly preferred.
  • 3-5 years post-Master’s experience in working with the homeless, mentally ill or elderly.
  • Supervisory experience preferred.
